Booking Exams and Specialist Visits
All ambulatory services, with the exclusion of laboratory tests, delivered through both the National Healthcare Agreement (S. S.N.) or on a Solvency (direct billing) basis require an appointment that can be booked through the channels described below.
Please have ready all the following documentation while making the appointment:
♦ Request from your family doctor or specialty doctor
♦ Healthcare card (only for services covered by SSN)
In case of “urgent but deferrable” over time, appointments can be made either directly at the CUP window or by sending a fax at the number 0331.329944 within 48 hours from the date of issue (excluding Saturday, Sunday and holidays) of the prescription with the green stamp.
For certain tests requiring specific preparation procedures, comprehensive information will be provided when booking.

dedicated section where any of the ambulatory services delivered by our organization can be booked.Simple instructions will guide you through the request procedure. CUP staff will contact you within the following 24 hours to confirm the appointment.

Rates
For services delivered with the coverage of the National Health Service patients will be required to pay only the “ticket” if applicable.Fee based services with no insurance coverage will be charged according to the rates applied by Gruppo MultiMedica available for viewing at CUP windows.
Insurance coverage and plans
MultiMedica accepts a broad range of healthcare coverage plans with Insurance companies, Funds of various nature, and companies. Lists are available for viewing at the Customer Information Service located in the “Entrance”.

General Information
The day of your appointment, Information Point staff will address you to the appropriate window where you will be able to register your personal information (when accessing our facilities for the first time), be checked in and pay for the service.
Documents required at check-in:
If the ambulatory service to be performed has been booked under the SSN Healthcare agreement, the documents to be presented to the check-in staff are:
♦ Family doctor request
♦ social security number
♦ Valid Identity document
♦ Healthcare card and Carta Regionale dei Servizi Sanitari
♦ Exemption documentation if available
If the service has been booked on a Solvency (direct billing) basis or if you have insurance/fund coverage, the required documents at check-in are:
♦ Specialty doctor prescription [request] (mandatory only for instrumental diagnostic services).
♦ social security number
♦ Valid Identity document
♦ Card or documentation stating your membership to the Gruppo Multimedica accepted Entity providing financial coverage
Personal and/or health data treatment consent
You will be asked to consent to:
♦ treatment of personal data by MultiMedica Group for healthcare and administrative purposes according to art. 13 of LD 196/2003
♦ Collection, registration and storage of personal medical records and/or biological specimen on behalf of MultiMedica Group for prevention, diagnoses, treatment and research
Under-age patients
Under-age patients must be accompanied by a parent or guardian; if this should not occur we will be unable to deliver the requested service. The accompanying adult will be asked to sign the personal data treatment consensus and a cumulative statement to self-certify their kinship. If the under-age patient is accompanied by a relative who is not the legal guardian, a written and signed authorization from the legal guardian will also be required.
Patients unable to sign Minors, temporarily disabled or illiterate patients
For all above cases, the documentation must be signed by an escort who will have to state his/her kinship to the patient.
Issuing and delivering results report
Once the check-in procedure has been completed, you will be asked to go to the waiting room of the clinic where the service will be performed/delivered. With the exception of services listed in the paragraph “deferred reporting”, results reports will be delivered immediately after the service has been completed.
NB: To simplify the physician’s activity in formulating a diagnoses and appropriate treatment when required, we suggest you bring with you all documentation related to previous tests/examinations (medical records, results, X-rays, etc.).
Deferred reporting
For certain services (e. g. Radiology services, BMD, lab tests, Holter, cytological and/or histological exams) the results report cannot be delivered immediately after the exam.You will be given a form specifying when you can pick-up the results.

Results will be delivered in a sealed envelope only directly to the patient involved, who will be asked to show personal identification and the results retrieval form. If this is not possible, the patient can delegate the pick-up to someone else by filling in the form included in the retrieval form. The operator is not authorized to deliver the results to anyone else without a signed delegation form. Please be aware that if results are not retrieved within thirty days from the service or specialized examination performance/delivery date, patients will be charged for the entire cost of the procedure (art. 5 comma 8 law 407, December 29 1990) ; except for cases that, for their nature, require over 30 days to be completed.
