Pre-admission
Pre-admission is a detailed diagnostics package “tailored to you” and includes sets of tests aimed at evaluating your condition prior to a scheduled surgical procedure; it is divided in two days.

How to prepare for pre-admission
Diet
The evening before pre-admission it is advisable to eat a light meal, avoiding all alcoholic beverages (wine, beer, liquors). No food should be consumed after midnight: only water may be ingested.

Medication
No medical therapy should be interrupted or suspended. Medication must be taken regularly at the usual hours even the morning of the exams.

What you should bring
The admission documentation required includes: 
     ♦ Personal identification 
     Codice fiscale (Social Security) healthcare/insurance card (if admission is covered by other care plans)
       Tessera sanitaria/assicurativa (se trattasi di ricovero convenzionato con altri enti) 
     If the patient is admitted through the SSN: referral/request from family physician/general care 
      provider
      
with admission information
        The referral must be delivered to the admission registration office at the latest when being admitted  
     If the patient is a citizen of the European community, in Italy temporarily: Communitarian certificate
        (for example: Form E111)
and the medical hospitalization prescription on the “carnet della salute” (health 
        book) issued by the locally qualified ASL    
     
Medical records 
and tests performed in the last two years (blood, sonograms, x-rays).



Pre-admission check-in
When entering the facility you will be required to register with the Pre-admission Check-in office; our staff will welcome you, collect your personal information and provide all information regarding your diagnostic track. You will also be 
asked to sign some forms which will be described in every detail..



  The Diagnostic Track
First Pre-admission Day
During the first Pre-admission day the following health evaluation tests will be performed - patients are requested to remain within our facilities throughout the procedure: 
     ♦  IRCCS in Sesto S.G.: from the scheduled appointment time, to 10.30 am at the latest
     ♦  MultiMedica Castellanza Hospital Facility: from the scheduled appointment time to 11.30 am at the
         latest.

Blood sampling
The diagnostic program starts with the collection of a blood sample, which completes the collection of biological samples and starts the laboratory tests.

Breakfast
After the blood sampling, you may have your breakfast at the hospital’s cafeteria, prior communication to our staff.

Electrocardiogram (EKG)

An EKG will be performed to verify the presence of cardiac rhythm disorders or muscle tissue distress caused by coronary vessel disorders.

Cardiac Examination
If required, it is performed to evaluate the presence of cardiovascular diseases based on the EKG record and the clinical heart exam.

Imaging Tests
     ♦  Chest X-ray: if requested, it will be performed to highlight any lung or cardiac diseases
     ♦  Other X-rays: specific pathologies (i.e. hip replacement, prosthetic knee, etc.) may require  
      additional radiology tests


Second Pre-admission Day
The time gap between the two Pre-admission days will depend on the time required to collect all the results of the tests performed.
The second Pre-admission day requires the patient’s presence in our facilities until the anesthesiology examination has been performed, and unless an additional evaluation examination is required from the medical team staff that is organizing the admission:
     ♦  IRCCS in Sesto S.G.:  from the scheduled appointment time to 2 pm at the latest    
     ♦ 
 MultiMedica Castellanza Hospital Facility:   additional tests to better understand any 
       clinical issues that may have emerged.

Conclusive anesthesiology interview
Only for patients receiving specific instructions, the pre-admission process will end with an interview with the Anesthesiologist. 
During the interview, the physician will: 
     ♦  fill in an informative questionnaire
     interpret i test results in real time, including comments on any clinical issues or possible risk 
      factors 
     request that you sign an anesthesia consent form, illustrating the most adequate 
      anesthesiology procedure for the specific case 
     
     ♦
provide
information regarding the security criteria adopted for same-day surgery or procedures
     ♦ prescribe additional tests to better understand any clinical issues that may have emerged

Pre-deposit blood performances
If requested, the hospital admission staff will inform the patient on the date established for the pre-deposit.




  Information and cancellations
Performed pre-admission care services are included in the patient’s medical chart: medical records will be collected and sent to the admissions department on the day of the patient’s entry. 
Patients will also be asked to sign a financial responsibility statement for the procedures performed in the event that hospitalization should not be confirmed for either health reasons or the decision to renounce. Admissions must occur within 30 days after pre-admission .
