AMBULATORIES 

Booking tests and specialized examinations 
All ambulatory services, both solvent and covered by the S.S.N., require a reservation through the channels indicated below, with the exception of laboratory tests.
To ensure correct booking procedures, be sure to have the following documents ready when contacting us:
     ♦ Referral from family doctor or request from specialized physicia  
     ♦ Healthcare card (libretto sanitario – only for services covered by the S.S.N.)

In case of procedures classified as “urgent but deferrable” over time, you may proceed to the CUP desk or sending via fax (02.24209300) within 48 hours from the issue date (excluding Saturday, Sunday and holidays), the prescription with the green stamp. For certain exams requiring specific preparation, detailed information will be provided when booking

Booking through the Web 
Through a dedicated area, it is possible to book all ambulatory procedures delivered by our facilities.
Simple instructions will guide you through the request procedure and our CUP (Booking Center) staff will contact you within 24 hours to complete the reservation.

Fees
For procedures covered by the S.S.N. the user will pay the ticket fee when applicable. For procedures and services delivered privately MultiMedica Group fees will be charged; rates are available at all CUP windows.  

Healthcare Agreements
 
MultiMedica has established agreements with many private insurances, funds, and companies; a complete list is available for consultation at the Customer service desk located on the ground floor.

Prevention Service and Check-Up

Available Monday to Friday from 8 am to 4 pm.

The service includes prevention packages, company check-up packages and customized check-up procedures based on individual requirements.
Performance procedures require mandatory booking (tel. 02/24209266-9267) and delivery must be completed within a day with the assistance of dedicated staf

Ambulatory Procedures Admission
General Information
The day of the appointment / procedure “Information” point staff will direct you to the appointed floor and desk to register your personal information, booking admission and paymen

Required documents for admission:

If the ambulatory service to be performed has been booked under coverage of the National Healthcare Service, the documents that must be presented to the check-in staff are
     ♦ Family doctor referral
     ♦ Codice fiscale (equivalent of social security number) 
     ♦ Valid Identity document 
     ♦ Healthcare card 
     ♦ Exemption documentation if available

If the procedure to be performed has been booked under Solvency terns or under cover of a private care/insurance plan, the documents to be presented are:
     ♦ Specialized physician referral (required only for instrumental diagnostic procedures) 
     ♦ Valid Identity document 
     ♦ Codice fiscale (equivalent of social security number) 
     ♦ Affiliation card to a program/Institute accepted by MultiMedica Group

Personal/confidential data treatment consensus:
You will be asked to consent to: 
     ♦ Treatment of personal data by MultiMedica Group for healthcare and administrative purposes according to art.13 of LD 196/2003 
     ♦ Collection, registration and storage of personal medical data and/or biological materials on behalf of MultiMedica Group for prevention, diagnoses, treatment and research 

Under age Patients
 
Under age patients must be accompanied by a parent or guardian: if this should not occur we will be unable to deliver the required service. The accompanying adult will be asked to sign the personal data treatment consensus and a cumulative certification statement to self-certify the kinship.
If the under age patient is accompanied by a relative who is not a legal guardian, a written and signed authorization from the legal guardian will also be required.

Patients unable to sign: minor, temporarily diabled patients, illiterate patients 
For all the above, the documentation must be signed by the escort who will have to state his/her kinship with the patient.

Once the administrative procedure is complete, the patient will be escorted to the care service waiting area.  With the exception of services listed in the “deferred reporting” paragraph, reports will be delivered immediately following the test

NB: To simplify the doctor's activity in establishing a diagnoses and, if necessary, appropriate treatment, we suggest you bring with you all documentation related to previous tests/examinations (results, X-rays, etc.)

Deferred Reporting:
For certain tests (i.e. Radiology, BMD, lab tests, Holter, cytological and/or histological) the report is not delivered immediately following the test.
You will be given a form specifying when you can pick-up the results.

Results in a sealed envelope must be retrieved directly only by the patient, who will have to show a valid ID and the results retrieval form.
If this is not possible, the patient can delegate the pick-up to someone else by filling-in the delegation section included in the retrieval form.
The operator is not authorized to deliver the results to others without the delegation form
Please be aware that for results not retrieved within thirty days from the care service or specialized examination date patients will be charged for the entire cost of the procedure (art. 5 comma 8 law 407, December 29 1990); except for cases which, for their nature, require over 30 days to be completed. .
